General
Integrative Evaluations does not conduct forensic evaluations for the purposes of court proceedings, fitness for parenting, risk assessments, or disability determinations, among others.
Integrative Evaluations does not provide treatment. However, we can recommend local clinics and appropriate treatments.
Clinical Intake Appointments
If you miss your intake appointment, we will likely cancel or reschedule your evaluation, as the missed appointment may prevent us from obtaining the necessary information to conduct the evaluation. If your evaluation appointment is canceled due to your missed intake appointment, your evaluation may be rescheduled for a later date, which can be several months later.
Neuropsychological Evaluations
If you are using one of the limited insurance providers that are in-network, it is your responsibility to call and confirm that your coordination of benefits (COB) is up to date. If your insurance provider informs Integrative Evaluations that the COB needs updating, we reserve the right to cancel and reschedule your appointment(s) until you update your COB.
Psychoeducational Evaluations
At Integrative Evaluations, psychoeducational evaluations are combined with neuropsychological evaluations to identify ways in which underlying neurocognitive or neurobehavioral disorders may be contributing to deficits or problems with one’s learning. Individuals who have had neuropsychological testing through Integrative Evaluations within the past year may elect to have add-on psychoeducational testing to rule out specific learning disorders.
Guardianship Evaluations
Insurance does not tend to cover Guardianship Evaluations, so we offer a private pay option.
Insurance Information
Integrative Evaluations is not in-network with most commercial insurance plans. It is your responsibility to contact your commercial insurance provider to find out whether they will approve an exception for out-of-network benefits and reimburse you for money paid toward the cost of the evaluation. We can provide you with a superbill, by request, that you can submit to your commercial insurance provider for reimbursement. A superbill shows an itemized list of services that you have received from an out-of-network health care provider. It’s like a detailed invoice that allows you to seek reimbursement. You pay for the services up front and then submit the superbill to your insurance provider. HRA/HSA and flexible spending accounts can also be used to pay for services. Integrative Evaluations does not request reimbursement from your commercial health insurance plan.
